Registration / Enrolment 

 Aspiring Medical is part of the WellSouth Primary Health Network. If you chose to register and enrol with us, you will be eligible for fees subsided by the Ministry of Health. You can only be enrolled at one medical practice and you must meet the government eligibility criteria for enrolment, outlined below.

In order to register and enrol with us, we ask you to complete an enrolment form. Our preference is for us to have your previous medical records transferred to us, to enable continuity of care. There  is a section on the enrolment form for this.  Please note we can only transfer medical records from practices within New Zealand.

We also require you to complete a Health Questionnaire, which helps us to update your file with your health information.

Once we have processed your Enrolment form and Health Questionnaire we will email you to let you know that your registration is complete.

A separate enrolment form is required for each family member.

If you do not attend the practice for three years, we are required by the Ministry of Health to ask you to complete a new enrolment form.

Eligibility Information 

 You are eligible to enrol with the Practice if you meet the following criteria:

  • You are a New Zealand citizen
  • You hold a New Zealand resident or permanent resident visa
  • You hold a work visa/permit that is valid for a minimum of two consecutive years
  • You are an Australian citizen or Australian permanent resident and can demonstrate that you have been, or intend to be in New Zealand for a minimum of two consecutive years

Patient Fees

Our fees vary according to the length and type of consultation, materials used, level of Government subsidy, your age and your residency status.

If you would like to clarify the cost of your consultation in advance, please contact our reception team on 03 443 0725.

A standard consultation is 15 minutes.

Different charges apply for weekend clinics, after hours, home visits and extended consultations. 

*Medical charges are for a standard (15 minute) consultation. There maybe additional treatment costs involved with your consultation please ask the Clinical team what they are likely to be. 

** ACC charges are the minimum consultation fee, additional charges will apply due to the nature of the injury and the treatment time.

Appointments

As an enrolled patient, you can make appointments through our online patient portal, ManageMyHealth. Once you have activated your profile in ManageMyHealth, you can also use this portal to request repeat prescriptions, view test results and access your clinical notes.

You can also make an appointment by phoning our friendly reception team on 03 443 0725, during clinic hours. Standard appointments are 15 minutes.

Please note, payment for your appointment is required on the day. If you feel that you may need longer than a standard 15-minute consult, please book a double appointment. Extended appointments are needed to discuss:

  • Multiple problems
  • Initial sports medicine assessments
  • Some mental health problems

Please discuss your needs with our friendly reception staff when you make your appointment. There is an additional charge for longer appointments.

Urgent Appointments

In an emergency, please call 111.
During clinic hours, we will always do our best to fit in patients with urgent needs. Please tell our reception team if you are requiring  urgent care. There is a charge for our urgent care service whether this is a phone or face to face consultation. 
Afterhours, our phone diverts directly to an experienced nurse triage team who can assist you, and coordinate call-out of the duty doctor and/or ambulance if required.

Cancellations and Missed Appointments

Please phone our reception team on 03 443 0725 if you cannot attend your appointment. At least two hours’ notice is required, as this may allow us to reuse your appointment time. If you cancel your appointment within 2 hours of the appointment time, or fail to attend appointments without notifying us or arriving late for you appointment, you will be charged the full appointment fee. 

Overdue Accounts

Payment for your appointment is required on the day. In the event you are having difficulty in paying your account, please have a chat to our reception team who, under exceptional circumstances, can organise a payment plan for you. Our bank account number is 06-0949-0380265-00 please put either your name or the name of the patient you are paying the account for in the reference field. 

Accounts that remain unpaid after 3 months will be passed onto a collection agency. Additional fees may be incurred for this service, that will be payable by the patient.

Privacy

Your privacy is important to us. All information regarding your care at Aspiring Medical Centre is confidential. In the course of your treatment, we may need to provide your details and medical history to specialists, hospitals, and other health providers. We ensure that your health information is seen only by those who are involved in your healthcare.

For funding purposes, we are required to provide some identifiable data to other health organisations. If this includes any personal health information, we will seek your consent. Under the privacy laws, doctors may also pass information to other health organisations to be used in a non-identifiable manner for health statistics.
